About the Pediatric Environmental Home Assessment Forms

 

General Guidance:

Each on-line form is designed to capture key information from the assessment.  After reading the scenario, and viewing the video clips and the photos, please complete the form by check-marking the boxes and filling in the blanks.  Please add notes in the "Notes" box to clarify your answers.

 

Complete each form, including the section requesting some limited information about yourself, then click on .  Each time you press , you will get a webpage confirming your form submission.  Judith Akoto will get an email providing her with your answers.  If she has any questions, she will get back to you with the results within ten business days. 

 

Try to click only once for each form.  If you want to change your information, then resubmit it and explain the changes in the Notes Box.  You do not need to complete the forms in the order listed.

 

Each on-line form contains a box identical to the one on the right.  It allows you to jump to any of the forms, videos, and photos.  You can also jump to the scenario, training start, and this hints page.  You should be able to follow the link and return to the on-line form without losing the boxes you checked and the notes you typed in. 

 

Hints:  

  • If you are impatient and want to see how you did, click here for a completed PEHA Survey Form and completed PEHA Nursing Care Plan that we think best matches the scenario. 

  • The information may not necessarily be presented in the same order as is found on the form.  For example, as you view the video clip for the kitchen you may observe information that applies to another section of the form (i.e. pests).  As with most home visits, your observations do not always occur sequentially in keeping with your paperwork.  You may need to view video clips more than once in order to capture all of the necessary observations.

  • The client's comments may contradict your observations.

We are constantly learning from your feedback.  Please use the "Notes" box in each section to help us understand how to do better and to identify environmental health problems we may have missed.  You are always welcome to send us an email.

Using the On-Line PEHA Survey Form:

We divided the three-page PEHA Survey Form into two on-line forms:  Survey Part 1 and Survey Part 2. Otherwise, the on-line forms look the same as the hard-copy form.  You can correlate the on-line forms to the sections of the hard copy Survey Form as follows:    

Potential problems are in bold.  Click on bold text to go to the section of the Nursing Care Plan that explains the actions we recommend you take to assist the resident. 

 

If you want to know why an issue is important, click on the Section heading (i.e. Indoor Pollutants) or the text in in the left column to get more information. 

 

Using the PEHA Nursing Care Plan:

We divided the seven-page PEHA Nursing Care Plan into three on-line forms.  Nursing Care Plan Part 1, Nursing Care Plan Part 2, and Nursing Care Plan Part 3.  Otherwise, the on-line forms look the same as the hard-copy Nursing Care Plan Form.  You can correlate the on-line forms to the sections of the hard copy form as follows:    

 

The first two sections of the PEHA Survey are on Part 1.  The remaining four sections are on Part 2.
